Consent Forms
The following consent forms must be signed (electronically) for EACH child that comes to the Gryphon. Parents/guardians can do this by logging on to Arbor and clicking on the ‘View Student Profile’ section. From here you will be able to give consent for each of the following areas. Please read each document carefully before consenting. The documents are linked below.
